By attending this event, I acknowledge and agree to the following terms on behalf of myself, my dog(s), and my guests:

• Leash Policy: My dog(s) will remain on a leash at all times while at BOSLY’S BACKYARD, INC.

• Dog Behavior: I confirm that my dog is sociable and has not exhibited aggression, threatening behavior, or harm toward any person or animal. I understand that other dogs will be present and that minor injuries such as cuts or scratches may occur. If my dog displays aggressive or otherwise unacceptable behavior, Bosly’s Backyard reserves the right to ask us to leave without a refund.

• Liability: I accept full responsibility for any harm or damage caused by myself, my guests, or my dog(s) at or in the vicinity of 4011 N Ravenswood, Suite 108, Chicago, IL 60613.

• Health & Vaccinations: I confirm that my dog(s) is/are in good health, up to date on Distemper/Parvo and Rabies vaccinations, and not suffering from any contagious illness.

• Waiver & Indemnification: I hereby release and agree to hold harmless Bosly’s Backyard, Inc., Kimberly Theobald, and its owners, officers, employees, and agents from any and all liability, claims, suits, or damages of any kind. I further agree to indemnify Bosly’s Backyard, Inc. for any loss, claim, injury, or damage caused by myself, my guests, or my dog(s).

• Photography Consent: I give permission for myself and my dog(s) to be photographed during the event. I agree that Bosly’s Backyard may use these images for promotional purposes, including on its website, social media pages, and marketing materials.
